Three Fairy Godmother Medals are used to evolve a 3★ Medal to 4★.
You can obtain Fairy Godmother Medals by completing the 3★ Evolve Medals quest in the ★-3★ Evolve Medals series of Special Quests on Fridays or by beating Guard Armor or Guard Armor Ω Raid Bosses.
Fairy Godmother Medals can also be obtained by defeating Pretenders in Quest 174, Quest 206, and especially Quest 270.
